What we collect
- Service inquiries: your name, email, phone, your relationship to the person seeking support, the county, the service you are interested in, how you prefer to be contacted, and any note you choose to write.
- Employment applications: your contact details, preferred work area and schedule, the position you are interested in, your experience and training, your answer about transportation, your resume if you upload one, and any accommodation request.
- Contact messages: your name, email, optional phone number, topic, and message.
- Site usage: privacy-friendly, aggregated visit statistics that do not use cookies or identify you personally.
What we ask you not to send
Please do not include medical records, diagnoses, Medicaid numbers, Social Security numbers, or financial details in any website form. If we need sensitive information later, we will collect it through a secure process designed for it.
How we use it
- To respond to you and follow up on your inquiry, referral, message, or application.
- To consider applicants for employment.
- To understand, in aggregate, how people find and use our website so we can improve it.
We do not sell your information. We do not send what you type into our forms to advertising or analytics companies.
Who can see it
Only authorized Haven Hues Wellness staff whose role requires it. Service inquiries and employment applications are stored separately, and each is visible only to the staff who handle that work. Our website host, database, and email providers process information on our behalf to run these services.
How we protect it
Information is encrypted in transit and stored with access controls and activity logging. Email notifications to our staff contain a reference number only, not the details you submitted.
How long we keep it
We keep inquiries and applications only as long as needed for the purpose you shared them and to meet our legal and record keeping obligations, then delete them.
Your choices
You can ask to see, correct, or delete the information you submitted through this site by contacting us. Please include your reference number if you have one.
Not an application for services
Submitting a website inquiry does not enroll anyone in services or determine eligibility, and it is not a Medicaid application.
